Dear community care access,


I am Darryl Goudreau making a referral into Long term care Housing or Retirement Home.

I need Intake.

I suffer from debilitating schizophrenia and have difficulty keeping up with day-to-day living at my home.
Meal preparation is to much to manage. I am no longer able to cook any of my own meals.
I need medication 4 times a day.
I have very frequent stays in the hospital.


I need comprehensive support.
I will need regular visits from nursing staff.
Moving into Log term-care is my only option.

It would be safer for me to live in a Long term care home and retirement home.
